New Student Registration

Instructions and Checklist for New Students

(Students who are currently not attending school in the Davis District)

Step 1 - Contact Shari Cloward to set up a registration appointment - 801-402-9058 or scloward@dsdmail.net 

Step 2 - Complete and organize the following documentation in preparation for your appointment

1. Photo Identification for the student's parent/legal guardian

2. An Original Birth Certificate for the student

3. A Complete Record of Immunizations or an exemption form printed and filled out at http://www.immunize-utah.org/

  • 5 DTP/DTaP/DT - 4 doses if 4th dose was given on/after the 4th birthday
  • 4 Polio - 3 doses if 3rd dose was given on/after the 4th birthday
  • 2 Measles, Mumps, Rubella (MMR)
  • 3 Hepatitis B
  • 2 Hepatitis A
  • 2 Varicella (Chickenpox) - history of disease is acceptable, parent must sign verification statement on school immunization record
  • 1 Tdap
  • 1 Meningococcal 

4. Proof of Residency - (including completed form) Residency form 

5. Student Information/Demographic Form - (complete with signatures) Info/Demographic form

6. Guardianship Status Form - (if items 2 through 5 are checked, legal documentation is required) guardianship status form

7. Review the New Student Checklist to make sure you have everything you need  new student checklist
